Questions to ask about stem-cell and cartilage-restoration treatment

Check the target condition, exact procedure and authorization, rather than the label.

Questions to ask about stem-cell and cartilage-restoration treatment

At a glance

  • Distinguish localized cartilage defects from extensive osteoarthritis.
  • Ask for the exact treatment and applicable authorization.
  • Check comparative evidence, cost, rehabilitation and failure plans.

The phrase cartilage regeneration covers different treatments. Surgery for a localized cartilage defect is different from a claim that an injection will reverse whole-knee osteoarthritis. Ask which problem in your knee is being treated, and how.

Three distinctions in regenerative treatment

  1. 01

    Target

    Localized defect or extensive osteoarthritis?

  2. 02

    Procedure

Which cells, product, injection or surgery?

  3. 03

    Evidence

    Authorization, human studies and rehabilitation?

The same label may refer to different targets and procedures.

Focal defect, exact treatment and applicable evidence

Separate a focal defect from whole-joint disease

A cartilage defect is assessed by location, size and surrounding cartilage and bone. Osteoarthritis can involve multiple compartments, alignment and ligament changes, so filling one area may not address the whole problem.

Restoration options also depend on loading and other injuries. Consider the tissue formed, patient pain and function, and long-term outcomes separately.

Ask for the exact treatment

Stem cells, bone marrow concentrates, cultured cell products and other biological preparations are not interchangeable. Ask about cell source, processing, injection versus surgery and any accompanying procedures.

The word regeneration does not establish restoration of the original joint or avoidance of replacement. Ask about comparable alternatives before committing to the cost.

Check country- and product-specific authorization

Authorization differs by country, product, indication and procedure. FDA warnings about US products should not be generalized into a claim that every treatment everywhere is illegal.

Equally, treatment at a hospital or listing in a trial registry does not prove effectiveness or appropriate authorization. Ask whether it is authorized for your diagnosis or is research participation.

Review outcomes and rehabilitation

Ask about human comparative studies, sample size, follow-up, adverse effects and further surgery. An improved image and meaningful pain or functional improvement are different outcomes.

Understand weight-bearing restrictions, rehabilitation and treatment if it fails. Claims of guaranteed cure or zero risk require scrutiny of evidence and consent information.

Who can receive bone marrow aspirate concentrate injections in Korea?

Bone marrow aspirate concentrate (BMAC) is prepared from a patient’s own bone marrow and injected into the knee joint. Korea’s new health technology notice specifies KL grade 2–3 knee osteoarthritis and administration as a standalone injection. Its recognized purpose is pain relief and improved function.

The effects recognized in the notice should be considered separately from claims about cartilage regeneration or long-term benefits. This procedure is currently a non-covered reporting item; new health technology recognition and health insurance coverage follow separate processes. Before treatment, ask about the expected benefit, bone marrow collection, and the procedure and material costs.

How does CARTISTEM differ from a routine joint injection?

CARTISTEM is a medicine containing allogeneic umbilical cord blood-derived cells. It is authorized for knee cartilage defects classified as ICRS grade IV caused by degenerative disease or repeated trauma. It is implanted into an exposed cartilage defect through an open or arthroscopic surgical procedure.

The ICRS grade of a cartilage defect and the radiographic KL grade of osteoarthritis are different classifications. The labeling lists KL grade 4, chronic inflammatory joint disease such as rheumatoid arthritis, and severe leg deformity judged unlikely to benefit among its contraindications. Eligibility is assessed using the defect’s location and extent, leg alignment and the severity of osteoarthritis together.

Common Questions

Are all stem-cell treatments alike?

Cell source, processing, product and delivery differ; effectiveness cannot be guaranteed across them.

Does authorization mean it works for everyone?

Authorization has defined indications. Individual benefit and risk still require assessment.

Does cartilage restoration remove osteoarthritis?

A focal defect and whole-joint osteoarthritis are different problems. Alignment, surrounding tissues and long-term course matter.

When to seek medical care

Each warning sign is a separate reason to seek care. Follow the action below if any one applies.

Emergency department · call 119 in Korea

  • Call 119 for sudden breathlessness or chest pain, severe faintness or loss of consciousness. Do not wait for calf swelling to appear.
  • Go to an emergency department if you suddenly cannot bear any weight, or if an injured knee swells rapidly or looks deformed. A cold or numb foot after injury also needs immediate assessment.
  • Seek emergency care for a hot, swollen and severely painful joint with high fever, shaking chills, confusion or inability to bear weight.
  • With back or leg pain, go immediately to an emergency department for new difficulty passing urine, loss of bladder or bowel control, or numbness around the genitals or anus. Severe or rapidly worsening weakness or numbness in both legs also needs immediate assessment.

Same-day assessment

  • Arrange assessment today for a newly hot, swollen joint, sudden severe pain, or a knee locked so that it cannot fully straighten. Joint infection can occur without fever.
  • Have new swelling or pain in one calf assessed today for a possible blood clot. Call 119 immediately if breathlessness or chest pain develops.
  • Severe knee pain or a new limp in a growing adolescent may come from a hip condition such as slipped capital femoral epiphysis. Stop walking and sport and arrange assessment today. Go directly to emergency care if they cannot bear weight.
  • Stop running and seek assessment today for new recurring groin pain, pain with walking or at night, or a limp. A femoral neck stress fracture needs to be considered. Keep weight off the painful leg until assessed. Go to emergency care if you cannot bear weight.

Book an outpatient appointment

  • If none of the warning signs above is present, book an appointment for pain lasting a few weeks, repeated swelling or reduced daily activity and exercise.
